Write the post you are applying for, your name in block letters, father's name, date of birth (DD/MM/YYYY), age as on 25/10/2026, address, mobile number and email. • Fill in the table of educational and other qualifications: degree/diploma, board/institute/university, year of passing, subject or specialisation, and percentage or division. • Fill in your employment details in chronological order: office/organisation, post held, temporary or permanent, period from and to, total experience, pay scale or monthly remuneration, and nature of duties. You may also add any further information that supports your suitability for the post. • Affix a recent self-attested passport size photo in the box on the form. • List every document you attach, with its page number.
