The form is also printed on pages 2 and 3 of the notification. • Fill in the post applied for, your personal details, category (SC / ST / OBC / Others), languages known, marital status and your age as on 14.11.2026. • Enter your education details (S.S.C., H.S.C., MBBS, Diploma, Post Graduation), your work experience and your NMC/SMC registration details. • Paste a recent passport-size photograph (3.5 cm x 4.5 cm). Tick "x" on any item that does not apply to the post, then sign the declaration. Unsigned applications will be rejected. • Send the form and documents by post to: Member Secretary (GB), Rogi Kalyan Samiti, Govt. Hospital, Daman Office of the Medical Superintendent, NAMO.
