Complete Bio-Data as per Annexure A-1 on ESIC website with a recent passport size photograph and one set of attested photocopies of testimonials (MBBS marks sheets & certificates, 10th certificate for DOB, category certificate, MCI/State registration, etc.)., Report to the office of the Medical Superintendent, ESIC Hospital, Silvassa Road, Chanod, Vapi, Distt.- Valsad (Gujarat) on 20 January 2026 before 11:00 AM., Bring original certificates/documents with one set of self-attested photocopies., Candidates claiming reservation must possess valid certificates in the prescribed format., Submit the application before 11:00 AM; applications received after 11:00 AM will not be entertained.
