How to Apply
• Fill the prescribed application form (Annexure C) attached to the notice; other formats or incomplete forms are rejected. • Paste a self-attested passport size photograph on the form. • Get a Demand Draft for Rs. 500/- in favour of 'ESI Fund Account No.1' payable at Chennai, if applicable, issued after the date of the advertisement. • Report for document verification on 17.11.2026 between 09.00 AM and 11.00 AM with the filled form, DD, original certificates and one set of self-attested photocopies. • Candidates whose documents are verified attend the walk-in interview. • A separate form and DD are needed for each additional department where more than one is allowed.
