The form is also printed on pages 5 and 6 of the notice. • Fill in the form and paste a self-attested photograph. Then sign the declaration. • Attach self-attested copies of the documents listed below. • Write "Application for the Post of Medical Officer under DMFT, Chatra" clearly on the envelope. • Submit the application by hand or send it by Registered Post/Speed Post to: The Deputy Commissioner-cum-Chairperson, 2nd Floor, District Mineral Foundation Trust (DMFT), Office of the Deputy Commissioner, Chatra, Jharkhand – 825401. • The application must reach the office by 18/11/2026, till 05:00 PM.
