• Take the proforma attached to the official notification (PDF). • Tick the post you are applying for and affix a recent passport size photo. • Fill in your name, age as on 01.10.2026 and date of birth. • Enter your Pension Payment Order (PPO) number, date of retirement, and designation and office at the time of retirement. • Give your total period of service in the cadre, permanent address, address for communication and contact number. • Sign the declaration that the details are true, and enter the place, date and your name. • Make sure the application reaches the office of the District Court, Alappuzha on or before the deadline.
