The fee is non-refundable. • Upload a proper signature and photograph with the online application; otherwise the application will be cancelled. • Complete the full submission of the application form by midnight of 26 November 2026. • Download the print copy of the online registration form from http://hr.wbhealth.gov.in/Index.aspx -> "Application Print" and keep it for document verification. Without it, a candidate will not be entertained for verification of original testimonials. • Keep the online registration number for future reference; the employer is not liable to give any information about it later. The hard copy or print copy of the online registration form does not need to be sent by post. Documents named in the notice: • Experience Certificate from the appointing authority (with the details listed above) • Caste Certificate issued by competent authorities of West Bengal (wherever applicable) • Print copy of the online registration form • Original testimonials for verification
