• Use the application form in the prescribed format attached to the notice ("Application for engagement of Project Human Resource Position, purely on temporary basis"). • Fill in every column in Block Capitals with a blue or black ballpoint pen. Write "N/A" where you have nothing to enter. Do not overwrite or use whitener. • Write your name, mother's name, father's name and date of birth (DD/MM/YYYY) as they appear in your Matriculation Certificate. Also give your category, your present address with PIN code, your own mobile number and your own email address. • Fill in your educational qualifications from 10th onwards and your work experience, both supported by documents. Also fill in your total experience after the minimum essential qualification, your research skills, your proficiency level in statistical software and the time you would need to join. • Paste a latest passport size photograph. Then sign the declaration, put your thumb impression in the marked spaces and fill in the place and date. • Send the completed form either by post to Room No 211, Second Floor, OPD Block, Department of Cardiology, AIIMS, Gorakhpur, or by email to [email protected]. Applications are accepted from 21 October 2026 and must reach by 25 October 2026. Late, delayed, incomplete or unsigned applications, and applications without a latest photograph, will be rejected without any correspondence. The institute is not responsible if an application is not submitted on time for any reason.
